
Douglas P. Sieminski, D.O.
Dr. Sieminski graduated from New York College of Osteopathic Medicine, Old Westbury, New York, in May 2003. He completed his residency training in Family Practice in June 2006 at Union Hospital (Affiliate of St. Barnabas Health Care System), Union, New Jersey, where he was the Chief Resident from 2004 to 2006. He also completed his internship at Union Hospital in June 2004. Dr. Sieminski attended Drew University, Madison, New Jersey, where he received a Bachelor of Arts Degree in Biology in May 1999.
Dr. Sieminski is Board Certified in Family Practice through the American Board of Osteopathic Family Physicians. He is a member of the American Osteopathic Association, American College of Osteopathic Family Practitioners, and New Jersey Association of Osteopathic Physicians and Surgeons.
Dr. Sieminski currently provides patient care at our Andover, New Jersey office.

Jennifer E. Horn, D.O.
Dr. Horn graduated from the University of Medicine and Dentistry of New Jersey, School of Osteopathic Medicine in Stratford, New Jersey in 1998. She completed her residency training in Family Practice in 2001 at Our Lady of Lourdes Medical Center in Camden, New Jersey. Dr. Horn attended The College of New Jersey in Trenton, where she received a Bachelors Degree in Biology in May 1992.
Dr. Horn was a Major in the United States Air Force, Pope Air Force Base in North Carolina from October 2001 thru October 2005, where she managed the clinic’s largest panel. She provided care to 12,400 patients.
Dr. Horn is Board Certified in Family Practice by the American Osteopathic Board of Family Physicians.
Dr. Horn has staff privileges in the Department of Family Practice at Saint Clare’s Hospital, 25 Pocono Road, Denville, New Jersey and Newton Memorial Hospital, 175 High Street, Newton, NJ 07860. She currently provides patient care at our Andover, New Jersey office.

Danielle Fortini, PA- C
Danielle Fortini, PA-C, earned her Master of Science in Physician Assistant Studies from Seton Hall University and graduated with honors from Rutgers University in 2018 with a degree in Biological Sciences. Coming from a family of educators, she developed a deep appreciation for lifelong learning and a commitment to patient education. Her passion for health, wellness, and disease prevention made a career as a physician assistant a natural choice.
With a strong foundation in urgent care medicine, Danielle has honed her ability to diagnose and manage a wide range of acute and chronic conditions. She is dedicated to fostering meaningful, long-term relationships with her patients, emphasizing preventive care and evidence-based decision-making to improve health outcomes.
Beyond her professional pursuits, Danielle values time spent with her husband, children, and grandchildren, finding fulfillment in both family and medicine.
Danielle is also certified to perform CDL physicals.

Cathleen R. Rizzolo, RN, BSN, MSN, ANP-BC
Cathleen R. Rizzolo, RN, BSN, MSN, ANP-BC graduated from the University of Medicine and Dentistry in Newark, New Jersey where she earned her Master of Science degree in Nursing in 2011. Cathleen earned her Bachelor of Science degree in Nursing at Bloomfield College in Bloomfield, New Jersey in 1999. She was awarded St. Luke’s Nurse Practitioner of the Year in 2014.

Leatrice Current, RN, BSN, MSN, APN-C. ANCC Board Certified Family Nurse Practitioner. Graduate of Fairleigh Dickinson University’s Family Nurse Practitioner Program in 2017. Lea has 18 years of Emergency Nursing and Primary Care experience.
